Let's start by creating the basic head shape using the Elliptical tool and holding down the Shift key to constrain.
Using the same tool in the same way, create the cheeks. For the eyes first create the white background and then make a copy of them and reduce the size and change the color to dark brown. For the highlight, create a smaller circle and move into place. When you are satisfied, select all of the eye parts and group them, then duplicate the eye and move into place.

For the nose, use the Pen tool and create the shape as shown below making sure to close the lines.
Use the Pen tool to also compose the mouth.
For the hat, select the Star tool and click in the board area to bring up the Options menu. Make the star have 3 points.
Use the Rotate tool to move the hat into the right position and then place the hat on the head.
Select Show Grid and turn on Snap to Grid. Use the Pen tool to create a design as the one below.
With the top piece selected open 3D and Revolve. Change the Offset to Right Edge.
Your Jack-in-the-Box should now have a body that looks like the one below. You can then send it to the back using Control and [.
Create two rounded rectangles with the Rounded Rectangle tool.
Duplicate the rounded squares and then apply gradient to three of the four. Apply a design to the most forefront one.
Use the Arc tool to create the handle.
Use the Pen tool to create the handle.
Use 3D and Revolve with Left Edge specified.
When you have completed this step, select Expand Appearance then use the Rotate tool to rotate the piece into the proper position.
Select the body and choose Drop Shadow. Change the color to a dark red.
Finally, group everything together, then choose Rasterize with a Transparent background and we're done.
