Create your label first and then drag it into the Symbols library.

Start by creating the 3D object by using the Pen tool and drawing half of the shape. Use the Direct Selection tool and select the two end points. Apply Path>Average>Vertical to the two end points to make them even horizontally with one another.
Change the Stroke to Fill. Use the Direct Selection tool here. Now select 3D, then Revolve.
Make sure to have Preview on and also select Right Edge for this to work correctly.
So now we have an object that we can map. Don't exit the 3D dialog box.
Select Map Art.
Make sure you have the new symbol selected. Use the Surface arrows to get to the right section of your object. Watch the actual object as you go through the choices.

Make adjustments to the placement here.
We have successfully place the artwork where it should be. If you are unsatisfied, just press Control+Z and start over. When all is done, use the Transparency Flattener to complete the project. Group the piece at this point and Save. Toss it into the Symbols palette to make sure it's safe.
