Using the Pen Tool, draw the shape shown.
Apply Revolve to the piece.
Rotate the cube 180 degrees and straighten.
The candle should appear once you press OK in the Revolve dialogue box.
With the Pen Tool, draw the shape of the flame.
Copy the flame, Control+C. Paste in front, Control+F. Use the Scale Tool set to 25%, to reduce the size of the second part of the flame. Change it's color too. Using the Selection Tool, move this piece down.
With both pieces selected, choose the Blend Tool. Select Specified Steps and set to 6.
Use the Blend Tool and click the exact same points on each, first one and then the other.
For the shadow, use the Ellipse Tool and create an ellipse approximately the size the wax catcher. Reduce the Transparency to 20% (the original color is similar to the candle).
Apply a 4 pt feather to it to soften.
With the Rectangle Tool, create a rectangle. Send the rectangle to the back, Control+[. To reduce the upper parts to create depth, use the Direct Selection Tool, select one point, hold the Control key down and use the right arrow near the keypad. Do the same for the other side. Use a slightly lighter color than the shadow.
With the table top selected, create a new gradient as shown below. Change the angle of the gradient to 90 degrees.
With the flame selected, apply an Outer Glow as shown below.
And here is the finished candle.
