Start with a black surface background. I'm using 5 x 7 measurement. Choose a round brush and increase it's size to 600 and make the Hardness about medium.
We'll start with a light blue color on a new layer.
Starting with the Brush tool, if you find there are gaps in the spacing when you paint, decrease Spacing located under Brush Tip Shape.
For the first stroke in blue, click once at the top, hold the Shift key down and click at the bottom. Change to white and click at the top. Use Control+[ to decrease the size of the brush and click again.
Apply Motion Blur set to -90 degrees at the maximum distance.
Reduce the size of the brush and increase the Hardness. Paint on a new layer.
Use Box Blur set to 75 and apply to white line. Then apply to blue layer 3 times.
Apply Transform using Perspective and make adjustments as shown below.
Select the tip area.
Apply the Glass filter set to Tiny Lens. Follow the parameters below.
The effect should look like this.
On a new layer create a word. Use Control+T to enlarge vertically. Then use the same command and choose Perspective.
Duplicate the word and move the top layer upwards. Select the lower layer and apply Box Blur. Move the top word down again.
Duplicate the layer again and move up a little. Apply a Box Blur again on the second layer. Use Transform>Perspective to adjust outwards slightly. Then use Transform>Scale to pull upwards.
Finally change the layer mode to Overlay.
