Here's the photo we'll use.

The first step is to create a snapshot and convert the picture to RGB, then duplicate the photo layer.

Open the Channel Mixer and set the colors to these percentages and hit OK.
Under Layer Mask choose Reveal All.
Create a circle with a feather of 65 pixels and anti-alias checked. Go to Layer Mask and hit Apply.
For the bottom later, create a new adjustment layer for Hue/Saturation.
Move Saturation to the left.
Your photo should now resemble the one below.
