In this tutorial I'll take you through the steps to create a memory look with color and tone.
Let's start by choosing a color photo.

Hit Control+J twice to duplicate the photo layer.

For the b & w layer, press Control+U Hue and Saturation, and slide the Saturation slider to the far left.

Change the mode of the top layer to Soft Light.

It'll look something like the photo below.

Still on the soft light layer, at the bottom of the layers palette click on the button to create a new adjustment layer. Select Color Balance.

Click on Highlights.

Move the slider left.

Next, select Midtones.

Move the Red and Yellow sliders outwards.

Change that layer's mode to Soft Light.

Reduce the Opacity to around 80%.

Click on the b & w layer and let's open up Lens Correction and under Vignette move the darken amount to the left and the Midpoint amount to about +75.
